Gender-Affirming Voice Care
Gender-affirming voice care helps a person bring their voice into line with their gender identity. It combines behavioural voice therapy with, where the patient chooses it, surgery that changes vocal pitch.
Your pathway
Assessment
Speaking pitch, range, resonance and stroboscopy.
Voice therapy
Pitch, resonance and speech patterns, practised over time.
Surgery, if chosen
Pitch elevation or lowering, after a full discussion.
Consolidation
Voice rest, then therapy to secure the result.
Overview
Voice is more than pitch. It carries resonance, intonation, and habit, and a durable result usually comes from working on all of these, not from surgery alone. Our approach starts with assessment and voice therapy, and considers pitch modification surgery as one option within a plan the patient leads, rather than as a default first step.
We treat this as clinical care, not a cosmetic package. The goal is a voice the patient experiences as their own, one that is comfortable and sustainable over long-term use. We are honest about what each route can and cannot change, and about the fact that surgical pitch change is difficult to reverse.
Who this is for
This care is for transgender and gender-diverse people who want their voice to reflect their identity, and for anyone seeking a change in habitual pitch. Some come for therapy alone, which for voice feminisation can achieve a great deal without surgery. Others come having already worked on their voice and want to discuss surgical pitch elevation. The plan is individual, and the first visit is an assessment, not a commitment to an operation.
Voice assessment
The first visit records where the voice is now: speaking pitch and range, resonance, intonation, and how the vocal folds look and vibrate on stroboscopy. We also talk about what you want your voice to do, in which situations, and what you have already tried. This assessment is the reference against which therapy and any later surgery are judged.
Voice therapy
Voice therapy is a course of sessions with a speech and language therapist, working on pitch, resonance, and speech patterns over time. Therapy produces gradual change that the patient maintains with practice.
Surgical options
Pitch modification surgery, when chosen, raises vocal pitch by changing the tension or vibrating length of the vocal folds; several techniques exist, and the choice depends on the individual. Surgery is followed by a period of voice rest and then therapy to consolidate the result.
Voice deepening (pitch lowering) is also available for patients who need it.
Follow-up
Surgery raises pitch, but the exact result cannot be guaranteed, and possible effects include some loss of volume or vocal range, especially at the low end. Surgical pitch change is not readily reversible, which is why we discuss it thoroughly before proceeding. We give an honest range of expected outcomes and are clear about the limits and the risks rather than promising a specific voice. After surgery, follow-up and therapy continue, remotely where the patient lives abroad.
Evidence and experience
Voice therapy for gender-affirming care is well established. Surgical pitch elevation has a substantial and growing literature, with results that vary by technique and by patient. We are candid that outcomes in this field are less uniform than for many other laryngeal procedures, and that the published evidence continues to develop.

Frequently asked questions
Can voice therapy alone feminise my voice?
For many people, yes. Voice therapy addresses pitch, resonance, and speech patterns together, and it can achieve a great deal without surgery. It is the starting point of our approach rather than a fallback.
What does pitch modification surgery do?
It raises vocal pitch by changing the tension or the vibrating length of the vocal folds. Several techniques exist, and the choice depends on the individual.
Is voice feminisation surgery reversible?
Not readily. This is why we discuss it thoroughly before proceeding, and why assessment and voice therapy come first.
What are the risks of pitch elevation surgery?
The exact resulting pitch cannot be guaranteed, and possible effects include some loss of volume or vocal range, particularly at the lower end. We give an honest expected range rather than promising a specific voice.
How long do I need to stay in İstanbul for surgery?
Pitch modification surgery needs a minimum of three days in İstanbul. We give you a specific plan, including follow-up, before you book travel.
A voice that feels your own
Start with an assessment. We will explain what therapy and, if you choose it, surgery could realistically change.
