R-CPD: The Inability to Belch

Retrograde cricopharyngeal dysfunction, or R-CPD, is the inability to belch. The muscle at the top of the oesophagus does not relax to let trapped air escape upward, so air builds up. The result is a set of symptoms that patients often recognise immediately once the condition is named.

How this program works

Your pathway

01

Remote review

Describe your symptoms; we confirm whether the pattern fits R-CPD.

02

Planning

Date, fee and a clear plan before you book travel.

03

Injection

Botulinum toxin into the cricopharyngeus, usually under brief anaesthesia.

04

Follow-up

Remote contact over the weeks while the reflex establishes.

What is R-CPD?

R-CPD is under-recognised. Many people who have it have never known it was a treatable medical condition, and some have been told the problem was psychological. Our first task is often diagnosis by recognition: hearing the pattern and confirming it. The treatment, when appropriate, is a targeted injection of botulinum toxin into the cricopharyngeus muscle, which relaxes it enough for the belch reflex to establish itself. For many patients the ability to belch, once learned, persists after the injection wears off.

We treat this as a defined condition with a defined treatment, not a rarity to be dismissed. We are also candid that R-CPD is a relatively recently characterised diagnosis, and that the evidence base, while growing and consistent, is younger than for long-established procedures.

Symptoms

The typical patient has never been able to belch, or lost the ability, and experiences some combination of:

  • a constant sense of trapped air and pressure in the chest or throat
  • painful bloating
  • gurgling noises from the throat
  • excessive flatulence
  • difficulty vomiting

Symptoms are often worse after meals, after fizzy drinks, and in social situations. Patients frequently arrive after years without an explanation, sometimes having found the condition described online before any clinician named it for them.

Diagnosis

The diagnosis is primarily clinical. A characteristic history, a long-standing or lifelong inability to belch together with gurgling and bloating, is usually enough, and examination of the throat helps to exclude other causes. Further tests are not routinely required; we use them when the picture is atypical or when another problem needs to be ruled out. For international patients, much of this history can be taken remotely before travel.

Botulinum toxin treatment

The injection is delivered into the cricopharyngeus muscle. At this institute it is usually performed in the operating room under brief general anaesthesia; for patients who strongly prefer it, an awake in-office approach is also offered. It takes a short time. After the toxin takes effect over the following days, most patients begin to belch, sometimes for the first time in their lives, often within a week or two. Some need to persist before the reflex establishes.

What to expect afterwards

The most common temporary side effect is some difficulty swallowing for a period after the injection, as the muscle is relaxed, along with the possibility of mild reflux of air or liquid. These usually settle as the effect wears off. A minority of patients need a second injection if the first does not fully establish the reflex. We set the expectation that this is a high-benefit treatment for a specific problem, not a guaranteed single-visit cure, and we explain the small risks before proceeding.

Follow-up

We stay in contact over the weeks after the injection, when the reflex is establishing. Most follow-up can take place by message or video, which suits patients who have travelled. If the reflex has not established after the effect has had time to develop, we discuss whether a second injection is appropriate.

International patient pathway

A large share of patients with R-CPD travel to us from outside Türkiye. The pathway is designed to be short:

  1. Remote review. You send a description of your symptoms; we confirm whether the pattern fits R-CPD and whether treatment is appropriate.
  2. Planning. We give you the date, the fee and a clear plan before you book travel.
  3. Visit. Assessment and injection usually fall within one to two days in İstanbul.
  4. Follow-up. Remote contact over the following weeks while the reflex establishes.

See International Patients for practical information about travelling to İstanbul.

Evidence and experience

Botulinum toxin injection for R-CPD is reported in a growing literature with high rates of symptom relief. The director has published on public and online information quality for R-CPD, reflecting an active interest in the condition.

FAQ

Frequently asked questions

What is R-CPD?

Retrograde cricopharyngeal dysfunction, or R-CPD, is the inability to belch. The muscle at the top of the oesophagus does not relax to release trapped air, causing bloating, chest and throat pressure, gurgling noises, and excessive flatulence.

Is the inability to belch a real medical condition?

Yes. R-CPD is a recognised and treatable condition. It remains under-recognised, and many patients have been told for years that the problem was psychological.

How is R-CPD treated?

With a targeted injection of botulinum toxin into the cricopharyngeus muscle, which relaxes it enough for the belch reflex to establish itself. For many patients the ability to belch persists after the injection wears off.

How soon after the injection will I be able to belch?

Most patients begin to belch within one to two weeks, sometimes for the first time in their lives. Some need to persist before the reflex establishes, and a minority need a second injection.

Does R-CPD treatment require general anaesthesia?

Usually yes. The injection is normally performed in the operating room under brief general anaesthesia. An awake in-office approach is available for patients who strongly prefer it.

How long do I need to stay in İstanbul?

A visit for an R-CPD injection usually needs one to two days in İstanbul. We confirm a specific plan for your case before you book travel.

Can R-CPD be treated in Turkey?

Yes. Istanbul Voice Center treats R-CPD with a botulinum toxin injection into the cricopharyngeus muscle, and a large share of our R-CPD patients travel from outside Türkiye. We start with a remote review of your symptoms, and the visit itself usually needs one to two days in İstanbul.
